Medical examinations for insurance purposes, pre-employment, certain driving licences and fitness to travel or participate in some sports are not covered by the NHS.

Certificates other than the standard capital DSS may include a fee. This also applies to reports to private medical insurance companies and the signing of passport forms.
Firearms and Shotgun Licence Medical Reports
Peartree Group Practice can provide a factual medical information report to support firearm and shotgun certificate applications and renewals.
Our Position
We provide factual medical information only.
We do not provide an opinion regarding whether an individual is suitable to own, possess or use firearms.
The Police Firearms Licensing Department is solely responsible for determining suitability and making licensing decisions.
What Information May Be Included?
The report may include:
- Information from your medical record.
- Current and past medical conditions.
- Relevant prescribed medication.
- Mental health history where recorded.
- Alcohol or substance misuse history where recorded.
- Relevant factual information obtained through discussion with you.

This is private work and is not funded by the NHS.
Payment must be received before work commences.
How to Apply
- Submit the required forms to the Practice.
- Provide written consent.
- Pay the £124 fee.
- A clinician will review your records and prepare a factual report.
